Registration: Present the front desk with your current insurance information, photo ID, and co-pay at each visit.

Late Policy: Patients who arrive 15 minutes after their scheduled appointment may be asked to reschedule. Every attempt will be made to accommodate sick visits on the same day.

Cancellations / No Show Policy: Appointments that need to be rescheduled or cancelled must give a 24-hour notice. We ask that same day appointments be cancelled within 2 hours of the upcoming appointment. This allows the appointment space to open for another child in need of care. More than three (3) last minute cancellations or no-show appointments may result in the termination of our medical relationship.

Sports / Daycare and School Forms: Well-child visits are the perfect time to get all upcoming forms completed. Forms that need to be completed outside of the well-child visit, can be dropped off at the front desk for completion as long as the patient has had their well-child visit within the year. Completion of these forms can take 24-48 hours.

Immunization Records: Vaccine records are available at any time for our patients.

Accompanying adults: Patients under the age of 18 must have a parent or legal guardian present at each visit. If a parent or legal guardian is unable to attend with the patient, another designated adult may come as long as written permission has been given from the parent or legal guardian. Permission forms can be obtained at the front desk.
